Unfortunatly, I tossed the box (which had wood reinforcement) before I decided to take a different route for my dash. LMC sells these for around $100 (with the nice box!) and has pretty good deals on shipping. I really doubt I'd be able to come up with a box and shipping that would be much better than them. Someone else asked the same question and I told them the same, plus, if I was off about LMCs pricing, to let me know and I'd put some numbers together for them. I didn't hear back from them, so, I'm guess they went with LMC.
Give LMC a call, it's a nice unit, I have it on 2 other trucks. Make sure you remove the existing dash pad from the truck completely, don't try to install the cover while it's in the truck. Glue the new cover on and use lots of "Blue" 3M tape to get it tight to the dash pad while the glue sets. Clean the wet glue from the edges and in a day, you're set. The first one I did about 5 years ago wasn't that great a fit. But, I think they improved their molds and got it right, I did one last year and it really looks nice.
Good Luck.
